FAFSA completion day

Creek Valley Seniors and parents are invited to participate in the FAFSA Completion Day on Feb. 25, 2013, at Sidney High School Guidance Office. EducationQuest Foundation, a free non-profit organization, will help complete and file your Free Application for Federal Student Aid (FAFSA).

Please call EducationQuest at 800-666-3721 to setup an appointment. Appointments are needed to receive assistance on this day. Individual appointment will be scheduled from 8 a.m. to 5 p.m.

Items to bring to your appointment include the following:

*PIN numbers for parent and student – apply at http://www.pin.ed.gov

*Parent and student 2011 federal tax returns, W-2’s and K-1 Form 1065 (if you have one)

*Value of checking/savings accounts and investments

*Amount of untaxed income: social security, child support, workers compensation

*Student’s driver’s license number

*Student’s Social Security number

*Alien Registration Number of eligible non-citizens

*Parent’s birth dates

*Student’s birth date

*Parent’s date of marriage, separation, or divorce
