Sports physicals available

Students planning to participate in sports, cheer, and danz during the 2016-17 school year you may get their physicals now at Sidney Regional Medical Center.

SRMC has the forms and they can also be downloaded from the school website. Please check to make sure all forms are filled out and signed by the physician, parents/guardians, and student before turning the forms in.

They can be turned into the high school office and need to be turned in by Friday, August 12, to start practice on Monday, August 15.
